Medical transcription has been around for a long time. In the 1950's and 1960s, hospitals would employ hoards of young women to do only transcriptions. They would sit in a large room with dozens of manual typewriters, tape recorders, and headphones, transcribing all the doctors' notes and observations.

All the patient rounds, consultations, and other correspondence were recorded and the tapes sent off to the "transcription department." There the tapes were converted into documents for insertion into patients' permanent medical records. If needed, these type written transcripts could be used for legal purposes. If it was written, then legally, it was said.
One would have thought that by now, there would be some other way of doing this, but alas, it is mostly still the same. Only now, the medical coders, transcribers, and other workers in that sort of field are contractors. This has opened up a whole new type of contract employee: the medical transcriptionist, and there are community colleges and other private institutions who train in this work, and whose graduates are widely sought after and valued.
Today, medical transcription services are on the Internet, and anyone needing the service can request a quoted rate for the services that are needed. Contractors who do this type of work are retained by the transcription companies, and they seek large contracts to provide their services.
Despite the advances of voice recognition technology, it still is not enough to overcome poor recordings, heavy accents, or a variety of other issues that might arise. Transcriptionists today use computer software that can slow down, rewind and replay, pause and so forth via a foot pedal control, allowing the hands to stay stuck to the keyboard for maximum efficiency.
These services have brought a lot of convenience to the client in this industry, the hospitals and other medical facilities. The services can be contracted with one contract and one point of contact for the entire facility or chain of facilities even. Having this contract labor available is a great value proposition to the client, and a great way for companies to get a lot of work with just a few large contracts in place.
However, it is uncommon for a single person to land a large contract with a large company. These are often given to corporations who then subcontract out to a network of employees that they have hired.
